Types of Breast Implants

There are three main types of breast implants available in the United States: saline, traditional silicone gel and highly cohesive, form-stable silicone gel. Each type of implant offers its own advantages and disadvantages. Dr. Mussat explains as follows:

Saline Implants

Saline implants have a silicone outer shell but are filled with saline, a sterile saltwater solution. One benefit of this type of breast implant is that the body can safely absorb the

filling material in case of an implant leak or rupture. Also, it is typically easier to detect a problem with a saline implant because the solution drains out of the implant shell, causing the affected breast to look deflated. In addition, Renowned Cosmetic Surgeon Dr. Mussat can use a slightly smaller incision when placing saline implants because she fills them with the filling material after they are inserted. Saline implants are also the most economical option.

One of the main potential drawbacks of saline breast implants is that they are more prone to visible rippling or wrinkling underneath the surface of the skin. For this reason, they are usually not an appropriate treatment option for women with thin skin or too little body fat to cover and support the implants properly.

Traditional Silicone Gel Implants

By far, the most significant advantage of traditional silicone gel implants is that they most closely resemble the look and feel of natural breast tissue. They are also less likely to wrinkle or fold within the implant pocket or cause visible rippling beneath the skin’s surface. Silicone implants are slightly more expensive than their saline counterparts, but most patients agree the benefits outweigh the cost.

The potential disadvantages of silicone implants are that the body cannot absorb the filling material, which generally stays intact either within or close to the implant pocket. Although this does not pose a visible risk, it does make it more difficult to detect a problem with the implant.

Highly Cohesive, Form-Stable Silicone Implants

The newest generation of silicone implants is more cohesive than traditional silicone implants. Thanks to their form-stable consistency, they are highly unlikely to fold or move around within the implant pocket even while the patient is reclining. In fact, form-stable silicone implants will retain their shape when cut in half. In addition, they come in a wide variety of shapes and different degrees of projection, often making them a good treatment option for women undergoing breast reconstruction surgery following mastectomy or lumpectomy for the treatment of breast cancer.

For some women, highly cohesive silicone implants feel too hard and do not resemble natural breast tissue as closely as traditional silicone implants do.

Incision Patterns

Depending on the patient’s aesthetic concerns and anatomical needs, Dr. Mussat will recommend one of the following incision patterns for breast augmentation surgery.

The Periareolar Incision

With this incision pattern, Dr. Mussat makes an incision around the outer perimeter of the areola, the darkly pigmented skin surrounding the nipple. The resulting scar is generally well concealed within the darker areola skin. However, this incision option may not be appropriate for women who are concerned with a temporary or permanent loss of nipple sensation and difficulty breastfeeding.

The Inframammary Incision

Dr. Mussat places this horizontal incision underneath the natural breast crease. This incision pattern offers little to no risk of affecting nipple sensation, but may be visible when the patient lifts up her arms without sufficient coverage of the breast (i.e., when wearing a bikini top or similar clothing). Nonetheless, with proper care, the scar should fade until it closely resembles the color of the surrounding skin so it is nearly imperceptible.

The Transaxillary Incision

This incision pattern is placed in the underarm area, in the armpit, making it almost invisible. It is best suited for the insertion of saline breast implants, before they have been filled by Dr. Mussat.

Placement Options

Dr. Mussat places breast implants either under the chest muscle (subpectoralis placement) or over the chest muscle but under the breast glandular tissue (subglandular placement). As with the other treatment options, there are pros and cons of each placement technique.

Submuscular Placement

Submuscular placement is appropriate for women with little body fat and thin skin, as it provides a greater degree of implant support and coverage. Nevertheless, this implant placement technique is more invasive than subglandular placement and may require a few more days of recovery time.

Subglandular Placement

This placement option is less invasive than submuscular placement and requires a less intense, shorter recovery period. However, it does not provide an ample amount of implant coverage and support. For women with thin skin and little body fat who undergo subglandular implant placement, the implants may be visible underneath the skin’s surface, making this a poor treatment option for them.
